If you have been scheduled for an appointment with one of our providers, please arrive 15 minutes before your first appointment with forms below completed. Questions about the forms can be addressed by calling the office at 904-335-3252 or at your initial appointment.

The Retreat at Ponte Vedra Beach Psychiatric Mental Health Services are out of network for all insurance companies. Payment is due at the time of service.
Forms of payment accepted include:
- Check
- Debit Card
- Credit Card
- HSA / FSA Card
All clients are provided with a superbill outlining their diagnosis, visit code, and charge that may be submitted to an insurance company for reimbursement.
- Initial patient appointments at The Retreat PVB are 60 - 90 minutes
- $400.00/session for 1-3 sessions depending on time needed to gather history
- Follow up appointments at The Retreat PVB are 30 - 60 minutes
- $200.00/session - $350.00/session
- Group Therapy (by acceptance) at The Retreat PVB are 60 - 90 minutes
- $125.00/session plus 1 time material fee of $25.00

Patient Portal
All families that establish care with The Retreat at Ponte Vedra Beach will be invited to communicate with their provider through our electronic medical record's patient portal: OnPatient. Through this patient portal you are able to view your upcoming appointments and check-in online, request schedule changes from your computer or mobile device, message your doctor through a HIPAA compliant portal, manage your contact information, access your portions of your medical record (labs and test results), have educational material sent directly to your account, and view billing statements.
